This page will have search criteria about the Client that pulls from the information that is already in the system.  If the client would like to select even more specific criteria about the provider agency click "Opt. Criteria".  For practice select "GROUP PSYCHOTHERAPY" by clicking the radio button beside it.  Then click "Opt. Criteria" at the bottom of the screen.

Special Note:  If you didn't need to select anymore search criteria you could select "Search" at the bottom of the screen to see a list of provider agencies that provide the selected service and meet the criteria specified.
