Consumer records provide a basis for planning
client care and for ensuring continuity of care by documenting the course of a consumer's
evaluation, treatment/habilitation and changing conditions; document communication between
responsible staff members contributing to the consumer's care as well as enable another
clinical staff member to assure care of the consumer at any time. Information is
disseminated only as allowed or required by state and federal confidentiality regulations.
